How to Collect Answers with a Form

Build a form, share it, review records and turn accepted ones into indicator totals.

A collection form is a set of questions your team answers, or people answer through a link. Accepted answers count into indicator values.

Design a form

  1. Open the project's Data tab and choose Collection forms, then New form.
  2. Name it. Add questions and choose each answer type: number, text, date, choice, yes/no, place, GPS point, or photo or document.
  3. For a question, you can tick Always required, or set Ask this only when or Make it required when based on an earlier question.
  4. Say which indicator the answers add up to: Count the records, or Add up a number from one question. Choose which date decides the reporting period (otherwise the day it was sent).
  5. Choose Save the draft, then Publish when the questions are right.

A published form cannot be changed. To change questions, close it and make a new form; earlier answers keep the questions they were given.

Answer a form

Open the form and choose Fill in. Answers that do not fit the question are explained beside it. Photos are JPEG, PNG, WebP or PDF, up to 5 MB.

Import many answers from a sheet

On Fill in, choose Download the sheet, fill one row per record, then choose a file or paste the rows and choose Check the sheet. If a row has a problem, nothing is imported; Download the rows to fix gives you only the rows that need correcting. Choose Import when the check is clean.

Review

On Records, accept a record or reject it with a reason. Return to waiting puts an accepted or rejected record back. Only accepted records count.

On What it adds up to, choose a reporting period and Enter as drafts. The totals become draft indicator values; review and submit them from the indicator values page.

Share with people outside the team

On Share a link, write who the link is for and choose Make a link. Anyone with it can answer the form without signing in. Withdraw stops a link.
