How to Record a Problem or a Risk

Keep a short list of what could go wrong, who owns it and when to look again.

Use this when something has gone wrong, or might, and the team should see it. On the project Overview, find Problems and risks and select Raise a problem.

The four things to fill in

  1. Has it happened? A problem has happened; a risk might.
  2. What is it? One short sentence.
  3. How serious is it? Low, medium or high.
  4. What are we doing about it? One short sentence, and who owns it (optional).

That is all. Open ones are listed with the most serious first.

Keep it current

Every open item is reviewed after 30 days; the owner is reminded. When it is over, select Close and say how it ended. Closed items stay in the list under See all.

Comment

Under each item, Add a comment lets the team discuss it. The owner and the person who raised it are told.

Problems and risks are for your team. They are never sent to the report writer.
